Please only complete this enrolment form if one of the following applies:

• You have met the general entry requirements for the Sixth Form and the subject entry requirements for your chosen courses (see entry requirements on the Sixth Form webpage).
• You have been contacted by Mr Ward because you narrowly missed the requirements for one or two subjects, have discussed your options with him following an appointment arranged through Mrs Davison, and he has confirmed that you may proceed with enrolment.

If neither of the above applies, please do not complete this enrolment form at this stage. Please contact the Sixth Form office (cdavison@chippingnortonschool.org) if you need further guidance.

External applicants: Before completing this form, please email a copy of your GCSE results statement to Mrs Davison, Assistant Head of Sixth Form (cdavison@chippingnortonschool.org).
